Interesting Sword, Possible Nagasaki Connection

After a break from collecting, I am re-enthused and back in the saddle, I cam across this today, with the interesting story it was presented to a Dr Glendinning for his work treating the victims of the two atomic bombs. but a story is just that.
The leather of the field cover is certainly shrivelled, Heat? maybe, maybe not. If anyone can help with translating the paper note attached it may prove or otherwise this fascinating story, likewise any help the the signature would be gratefully received.
